Access to medical records

The practice is registered and complies with the Data Protection Act 2018 (DPA 2018). Any request for access to notes by a patient, patient’s representative or outside body will be dealt with in accordance with the Act. Please contact the Practice Manager for further information.

 

In accordance with the Data Protection Act 1998 and Access to Health Records Act, patients may request to see their medical records. Such requests should be made through the Service Delivery Lead and may be subject to an administration charge. No information will be released without patient consent unless we are legally obliged to do so.
The forms to request this data are available outside of the building or by downloading and completing this form.
