Travel Vaccinations

FORMS MUST BE COMPLETED AT LEAST 6 WEEKS BEFORE TRAVEL OR AT LEAST 8 WEEKS FOR COMPLEX TRAVEL ITINERIES 

It is important to make your initial appointment for assessment as early as possible  – at least 6 weeks before you travel – as a second appointment will be required with the practice nurse to receive the vaccinations.  Some of these vaccines have to be ordered as they are not a stock vaccine.  Your second appointment may need to be at least 4 weeks before you travel to allow the vaccines to work.  Not all travel vaccinations are included in the services provided by the NHS.

Polio

Typhoid

Hepatitis A

Hepatitis A/B combined

Cholera

Tetanus

Please complete the Travel Health Questionnaire below for each traveller.  Once the form(s) is received a member of the reception team will contact you to arrange a telephone assessment interview with the Practice Nurse and a second appointment for any vaccinations that may be required following this assessment.  We will also send you a text with the following link and ask that you look up your destination prior to your telephone assessment.  This will help prepare you for the assessment.
